3. HOW TO ENTER: To participate in the Sweepstakes, you may enter as many times as you would like via the following methods:


a.	In-Restaurant. During the Sweepstakes Period, visit a Bertucci's restaurant and fill out a guest survey; you will be asked at the end of the survey if you would like to participate in the drawing. Click on the link and follow the on-screen instructions to complete the entry form to submit your entry. Limit one entry per guest check.

b.	On-Line. During the Sweepstakes Period, visit the Web Site www.Bertuccis.com and fill out a guest survey; you will be asked at the end of the survey if you would like to participate in the drawing. Click on the link and follow the on-screen instructions to complete the survey and entry form to submit your entry. Limit one on-line entry per person per day.

c.	Mail-in. Mail a hand-printed listing of your name, complete mailing address and a valid e-mail address or valid telephone number (including area code), on a postcard or separate piece of paper no larger than 5½" x 8½", to the "Mail-in Address" listed below. Mail-in entries must be postmarked by June 23, 2012, and received by Friday, June 29, 2012 at 4:00 PM Eastern Time. Limit one entry per postcard or envelope.

An eligible entry must include the entrant's name, U.S. state of legal residence, either a valid e-mail address or valid telephone number (including area code). You are providing this information to Bertucci's and not to Facebook. Sponsor and Administrator have the right to verify eligibility of each entrant and disqualify those who don't meet the eligibility requirements.

When you enter at a Bertucci's restaurant or through the Bertucci's web site, you may be asked to consent to receive promotional e-mails and reminders for upcoming Bertucci's promotions and information about Bertucci's. Consenting to receive such e-mails is optional and is not an eligibility requirement, and does not increase your chances of winning. In the event you do consent to receive these e-mails, the use of such e-mails shall be subject to the privacy policy of Bertucci's Corporation.

4. ELIGIBILITY REQUIREMENTS:


a.	The Sweepstakes is open to persons who are legal residents of the 50 United States and the District of Columbia, who are at least 21 years of age or older as of May 31, 2011. The Sweepstakes is void outside of the 50 United States and the District of Columbia, and where prohibited by law.

b.	If the entry does not contain the entrant's name, valid U.S. state of legal residence, and either a valid e-mail address or valid telephone number (including area code), the entry will be void.

c.	The following persons are not eligible to participate or win: Employees of Bertucci's restaurant locations in Massachusetts, New Hampshire, Rhode Island, Connecticut, New Jersey, Pennsylvania, Maryland, Delaware, Virginia, the District of Columbia, and New York, and employees of the restaurant locations' licensees, corporate parents (including their officers and directors and their immediate families), subsidiaries, and affiliates, and all of their advertising agencies; and the members of any of those employees' immediate families (parent, children, siblings, or spouse, regardless of where they live) or persons living in the employee's same households (whether related or not).

d.	Prize winners must sign (all in a form acceptable to Bertucci's): an Affidavit of Eligibility, an Assignment of Rights, a Publicity Release, IRS Form W-9, and a Liability Release without additional compensation (where permitted by law). Winners must execute these documents, without making changes, and return them for receipt by Bertucci's within fourteen (14) calendar days of notification. Failure to execute and return these documents within fourteen (14) calendar days will result in disqualification and forfeiture of prizes.

e.	Obscene, provocative or otherwise inappropriate content will disqualify an entry. Entries that otherwise fail to comply with the Official Rules will be disqualified.

f.	Two (2) Winners will be drawn at random from all entries received during each calendar month of the Sweepstakes period. To qualify for entry into the drawing for a given calendar month during the Sweepstakes Period, an eligible In-Restaurant or On-Line entry must be submitted no sooner than 12:00:00 AM on the first day of the calendar month and no later than 11:59:59 PM Eastern Time (EDT or EST as applicable) on the last day of the calendar month and an eligible Mail-In entry must be received by Bertucci's no sooner than the first day of the calendar month and no later than the last day of the calendar month-(or sooner as required for the month of June 2012).

7. WINNER SELECTION AND NOTIFICATION:


a.	Two (2) Winners will be selected in a random drawing from all eligible entries in the "Guest Survey Give Away" received during each calendar month of the Sweepstakes period.

b.	All Sweepstakes decisions and interpretation of these Official Rules are in Bertucci's sole discretion and are final.

c.	Bertucci's will select the Potential Winner(s) (each a "Potential Winner") from all eligible entrants received during a calendar month in the Sweepstakes Period. The random drawing for each calendar month will occur between the 1st and 15th of the month following each calendar month in the Sweepstakes Period. Each Potential Winner will then be contacted via e-mail or telephone using the information on the entry form. Bertucci's reserves the right to delay the selection of the Potential Winners.

d.	Bertucci's will notify each Potential Winner within ten (10) calendar days of selection, via e-mail or telephone using the information on the entry form. Three attempts will be made to notify each Potential Winner. The Potential Winner must respond to email or voicemail notification within 48 hours of the third attempted notification or he/she will forfeit the prize If Bertucci's is unable to contact a Potential Winner, Bertucci's will select an alternate Potential Winner by random draw. Alternate Potential Winners are subject to all requirements set forth in these Official Rules. A Potential Winner becomes a Winner upon Bertucci's determination that the Potential Winner has met all eligibility requirements, additional restrictions and conditions.

e.	Bertucci's will send the applicable Prize to each Winner within 30 days of becoming a Winner. In the event that the Prize is mailed, each Winner assumes the risk of its loss. Bertucci's is not responsible for the safe arrival of a Prize.

8. ADDITIONAL RESTRICTIONS AND CONDITIONS APPLY:


a.	By submitting an In-Restaurant, On-line or Mail-In entry, the entrant accepts all Sweepstakes rules and agrees to be bound by the decisions of Bertucci's. Bertucci's reserves the right to disqualify an entrant if it has reason to believe an entry is not valid or if the entrant refuses to be bound by the Official Rules.

b.	By participating in the Sweepstakes, where permitted by law, the Winner(s) agree to give Bertucci's full rights to use the Winner's name, voice, likeness and image in any medium for advertising, broadcasting, and publicity relating to this Sweepstakes, without additional financial or other compensation, and to sign a publicity release confirming such consent prior to acceptance of the Prize.

c.	Prior to awarding any prize or prize certificate, Bertucci's may require Winner(s) to sign a liability release, agreeing to hold Bertucci's, including but not limited to its related companies and licensees, each of their parent, subsidiary and affiliated corporations, and each of their respective officers, directors, shareholders, insurers, employees, attorneys, representatives, agents and assigns (the "Bertucci's Parties") harmless against any and all claims or liability relating to or arising directly or indirectly from the prize or participation in the Sweepstakes.

d.	All entries become and remain Bertucci's exclusive property and will be neither acknowledged nor returned.

e.	The Bertucci's Parties and their Internet servers or access providers are not responsible for lost, stolen, misdelivered, illegible, mutilated, indecipherable, altered, defective, incomplete, incorrect, inaccurate, postage due or late entries, due to human error or otherwise; entries that are delayed or are not received or not processed due to telephone, network, electronic or computer hardware or software failure, technical malfunctions or problems of any kind; or any injury or damage to entrant's or any other person's computer related to or resulting from participation or downloading of any material in this Sweepstakes. Proof of e-mailing and/or registering does not constitute proof of delivery or receipt of the entry or that Bertucci's actually received it by the entry deadline. Additionally, Bertucci's has no obligation to advise an entrant of an incomplete or otherwise non-compliant entry.

f.	As a condition of participating in the Sweepstakes, each entrant, on his or her own behalf and on behalf of his or her agents, servants, trustees, heirs, or permitted assigns, releases and holds harmless the Bertucci's Parties from any and all damages, injuries, claims, causes of actions, or losses of any kind resulting from participation in this Sweepstakes, including infringement of intellectual property rights.

g.	Entries will be declared to be made by the authorized account holder of the e-mail address submitted at time of entry. "Authorized account holder" is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider, or other organization (e.g. business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.

h.	This Sweepstakes is void where prohibited by law. As a condition of participating in the Sweepstakes, each entrant, including their agents, servants, trustees, heirs, or permitted assigns, agrees that: (1) all issues and questions concerning these Official Rules and the Sweepstakes shall be governed by and construed in accordance with the laws of the United States and the Commonwealth of Massachusetts, without giving effect to any choice of law or conflict of law rules; (2) should there be a conflict between the laws of the Commonwealth of Massachusetts and any other laws, the conflict will be resolved in favor of the laws of the Commonwealth of Massachusetts; (3) any and all disputes, claims and causes of action arising out of or connected with the Sweepstakes or any Prize awarded shall be resolved individually, without resort to any form of class or collective action, and exclusively by the appropriate court located in the Commonwealth of Massachusetts; (4) entrant waives all punitive, incidental, or consequential damages, including any attorneys fees, other than entrant's actual out-of-pocket expenses (i.e., costs associated with entering this Sweepstakes), and entrant further waives all rights to have damages multiplied or increased.

i.	Bertucci's is not responsible for typographical or other errors in the printing, the offering or the administration of the Sweepstakes, or in the announcement of a Prize.

j.	It is the entrant's responsibility to enter in the appropriate manner. Each entrant is responsible for all costs or charges involved in accessing the Web and any Web site(s) and warrants that the cost of accessing the site, if any, is part of a larger agreement with an Internet Service Provider or some other means, and that the cost of this access is not an incremental cost.

k.	Bertucci's operates full service restaurants, and values the safety and the positive experience of its patrons, guests, invitees and employees. Entrants and potential entrants agree that, in connection with the Sweepstakes, they will not damage, disrupt or interfere with the normal operation of any Bertucci's restaurant.

l.	Any attempt to deliberately damage, disrupt or interfere with the normal operation of Bertucci's Web site or undermine the legitimate operation of the Sweeepstakes may be a violation of criminal and civil laws and Sponsor reserves the right to seek damages from any such person to the fullest extent permitted by law.

m.	An entrant will be disqualified if Bertucci's determines in its sole discretion that the entrant has or has attempted to tamper with entries; has or has attempted to damage, disrupt or interfere with the entry process, the operation of the Sweepstakes, Bertucci's Web site, or any Bertucci's restaurant; or has violated the Official Rules.

n.	If for any reason any portion of the Sweepstakes is not capable of running as planned, including infection by computer virus, worms, bugs, malicious or injurious code, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor, which corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Sponsor reserves the right to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify, or suspend the Sweepstakes. Should any portion of the Sweepstakes be terminated prior June 30, 2012, Potential Winners will be drawn from eligible entries received up to the time of termination.
